A Troubling Trend in Indian Healthcare

Recent reports indicate that the majority of grievances filed by beneficiaries of the Ayushman Bharat scheme point to a troubling trend: hospitals demanding unexpected payments for treatments. This alarming issue contradicts the fundamental purpose of the scheme, which is designed to offer cashless hospitalizations and alleviate the financial burden of out-of-pocket expenditures (OOPE).

The Statistics Behind the Grievances

A report conducted by the Comptroller and Auditor General (CAG) in 2023 revealed that an astonishing 75 out of 121 healthcare providers who were de-empanelled between 2018 and 2023 were found complicit in illegal payment collections and other fraudulent practices. This confirms suspicions held by many beneficiaries regarding the integrity of the healthcare providers enrolled in the scheme.

State-Specific Challenges

An analysis from the Centre for Health Equity, Law, and Policy along with the Association for Socially Applicable Research sheds light on the regional disparities in grievances:

  • Top States for Service Quality Grievances: Chhattisgarh, Jharkhand, and Uttarakhand stood out for their complaints regarding the quality of hospital services.
  • Top States for Eligibility Issues: Maharashtra, Assam, and Bihar experienced the most significant challenges related to eligibility and registration complications.
  • Behavioral Concerns: Uttar Pradesh, Bihar, and Madhya Pradesh reported the highest grievances concerning the conduct of hospital staff and overall quality of services.

Nature of Complaints

The study revealed that a staggering 74% of complaints centered around hospitals requesting extra payments. Other identified issues include:

  • 15% concerning denial of treatment
  • 6% involving hospitals withholding Ayushman Bharat cards
  • 2.2% pointing to non-availability of medicines and diagnostic services
  • 2% expressing dissatisfaction with the quality of treatment provided

Registration and Eligibility Issues on the Rise

During the COVID-19 pandemic, grievances related to registration and eligibility for the Ayushman card emerged as the highest proportion of complaints. The study analyzed 1.1 lakh grievances filed through the AB-PMJAY grievance redress system from October 2018 to March 2022, uncovering that 67% of grievances fell into the ambiguous “other” category. This vagueness raises further questions about transparency in the grievance process.

Lessons from Assam: A Case Study

The incidence of grievances in Assam was notably higher than in other states participating in the AB-PMJAY program, with approximately 52% linked to eligibility and registration flaws. Delays in beneficiary registration, averaging 187 days, were highlighted as a significant issue, exacerbated by a 70% manpower shortage within the state health agency, as noted in the 2023 CAG report.

The Path Forward: Ensuring Accountability

As the grievances pile up, there’s an urgent need for policy reforms and more robust oversight mechanisms in the implementation of Ayushman Bharat. The scheme’s objective of offering accessible and affordable healthcare cannot succeed if the beneficiaries continually face financial demands from hospitals. Revamping the system could enhance trust and provide the quality care that the scheme aims to deliver.
